Rotherham’s Integrated Health and Social Care Place Plan 2018-20 aligns closely to the Rotherham Health and Wellbeing Strategy. It describes Rotherham’s achievements to date, future strategic intent and how the relationships between the health and social care community have successfully matured over the last few years

New initiatives:

  • Get Healthy Rotherham and Cultural Strategy - improving peoples’ physical activity levels by encouraging people to get active and outdoors.
  • The Rotherham Health App is a brand new service providing online access to manage healthcare 24 hours a day. It is available 24/7, on desktop, tablet, or mobile devices. Patients are able to book appointments, manage their medication, view test results, and access their medical record.
  • Make Every Contact Count (MECC) pilot underway –  loneliness action plan which supports the existing Better Mental Health for All strategy to support all communities in Rotherham to manage and look after their wellbeing through targeted interventions, wellbeing education and strengthening links with community and voluntary sector

System Plans and Developments:

  • A new single electronic referral process introduced - estimated to save around 30 minutes per patient, and also saved around half a million pounds of acute bed days.
  • Success of The Rotherham Maternity Transformation programme - Personalised Care Plans now being offered to 100% of women and the reduction of women smoking at the time of birth.
  • The Rotherham Health Record – a transformative electronic system for sharing patient health information in a safe, secure way. It reduces duplication for patients of going over their health background with each new consultation. The medical professionals using the notes can make the right decisions among patient’s wider health and care needs.

Residents report being in:

  • 43.2% very good health and 33.9% good health
  • 15.3% in fair health
  • 5.9% bad health and 1.7% very bad health
